Centacare Evolve Housing is excited to launch The Hub. The Hub will include an onsite Nurse Practitioner and a visiting General Practitioner who will provide a health clinic every Thursday.
What is a Nurse Practitioner?
Nurse Practitioners provide high levels of clinically focused care, including prescribing a range of medications and conducting clinical tests. They can be the primary provider of care or part of a care team. Nurse Practitioners are capable of managing episodes of care, including wellness-focused care, and investigative care and treatment.
Where is The Hub?
The Hub will be located in the Centacare Evolve Housing office – Shop 2, 28 Green Point Road, Bridgewater, TAS 7030.
How do I access services at The Hub?
The Hub’s health clinics will commence on Thursday 16 February 2017, and will be available every Thursday thereafter.
What if I still need to see a GP?
A GP will be present at The Hub’s health clinic every week. Patients who require further follow up and treatment with a GP after visiting The Hub can choose to be referred to one of All Round Health and Community Care’s visiting GPs, or to their regular GP.
Will my details be kept private and confidential?
Yes. When booking an appointment you will only need to provide the name and address of the person who is to be seen, no other information is required at booking stage. On the day of your appointment you will need to complete a new patient form that will request further information such as your Medicare number and any current medications you may be taking. All Round Health and Community Care will hold this information confidentially.
What do I need to bring?
Please bring photo ID, your Medicare/concession Card, details of your current GP, and details of your next of kin.
