Centacare Evolve Housing’s Client Support Program works with tenants who need support to sustain their tenancy.

These situations are often complex and need several services to work together with the aim of supporting individuals and families to maintain their tenancy and stay in their home.

The Program’s focus is on delivering better outcomes that contribute to individual, family, and community wellbeing. Our Support Coordinators provide outreach, intervention, case planning, and referral pathways that bring key services together.
